Dr. Nagasako focuses on ways to better address health-related social needs in medical settings, especially for vulnerable populations. Her projects adapt hospital data analytic and quality improvement tools to address community health needs and support partnerships between hospitals and community organizations to develop and implement new models of care.

Dr. Nagasako’s data analytics projects involve using hospital data and census data to evaluate the impact of social factors on hospital outcomes and to create a ZIP-code-level index of health factors and health outcomes to better target community health interventions. Projects in the area of process improvement and innovation use human-centered design to identify and address barriers to access in rural and urban settings and extend Lean quality improvement methods to safety-net settings in order to improve transitions of care for underserved patients.
