Trained members of our team will reach out to individuals affected by a violent injury at one of the participating hospitals. Individuals, family members and friends can also reach out to our team.
Step One
Initial contact is made through our COR Team at one of our participating hospitals.
Step Two
Our COR Team offers initial support and connects affected individuals to our case managers upon entry into the LOV program.
Step Three
The participant works with a clinical case manager to pave a path toward a brighter future through holistic support.
